We reviewed a prospectively maintained database and analyzed the following variables: patient demographics, intraoperative, surgical, and pathologic data, and perioperative complications. From 2003 to 2007, 355 patients underwent bilateral nerve-sparing radical prostatectomy. All patients had sponges placed in the surgical field. One hundred six patients (50%) had a suction sponge placed between the endopelvic fascia and the prostate, 69 (32%) had a suction sponge placed between the bladder and the endopelvic fascia, and 35 (16%) had both. Data were further analyzed by comparing the 1-sided Fisher’s exact test or t-test for categorical and continuous variables. The mean age was 62.7 years; 203 (56%) patients had pathologically organ-confined disease; and the median preoperative prostate-specific antigen level was 7.7 ng/mL. The incidence of complications was similar between patients with these 3 techniques. The incidence of intraoperative urinary extravasation was lower in the combination group than in the suction group (12% versus 17%, P = 0.03). Of the patients experiencing urinary extravasation, the incidence was similar among the 3 groups (P = 0.16). The incidence of intraoperative bladder injury was the same for the combination and suction groups (2% versus 3%, P = 0.69) but lower in patients with a suction sponge (1% versus 6%, P = 0.02). The overall incidence of postoperative urinary tract infection was lower in patients with a combination of sponges than in those with a suction sponge (5% versus 11%, P = 0.04). There were no other differences in complications or recurrence rates.
